Pre-Elementary
The school sets the requirements for pre-elementary applicants.
They must:
a. Submit Original PSA Birth Certificate
b. Baptismal Certificate
c. Pass the SJASQ Entrance Examination
d. Be at least 4-5 years old for Pre-Kindergarten/ 5-6 years old for Kindergarten *refer to Deped Order No. 47 s. 2016
e. ECCD Checklist for Incoming Kindergarten
f. Be in good health
g. Applicants who belong to another religion will be accepted after an interview with the parents and only if he/she is willing to sign an Agreement (Kasunduan) containing the rules to be followed while inside the SJASQ campus.
h. Applicant who is diagnosed with special condition and/or neurodiverse will be accepted after an interview with the parents and upon the presentation of diagnostic result coming from neuro developmental pediatrician.
Elementary
The following should be observed for admission and enrollment:
a. All enrollees should present Form 138/SF9 (Report Card).
b. A transferee should:
1. pass the Entrance Examination
2. submit PSA Birth and Baptismal Certificates
3. present a certification of good moral character or recommendation of the former principal.
*Acceptance of those with low general average will be dealt with on a case-to-case basis and should see the principal with his or her parents.
4. present a First Communion certificate – (for incoming Gr.4-6 only)
c. An applicant for Grade One should:
- be 6 to 7 years old
- be in good health
- present PSA Birth and Baptismal Certificates
- pass the Entrance Examination
d. Applicants who belong to another religion will be accepted after an interview with the parents and only if he/she is willing to sign an Agreement containing the rules to be followed while inside the SJASQ campus.
e. Applicant who is diagnosed with special condition and/or neurodiverse will be accepted after an interview with the parents and upon presentation of diagnostic result coming from neuro developmental pediatrician.
Junior High School
Entering Freshmen
1. Elementary Graduates of SJASQ
In order to be accepted in Grade Seven, each applicant should abide by the following enrolment procedures:
a. Submission of Form 138/SF9 (Report Card)
b. Applicants with a general average of 80% and above and with at least satisfactory conduct grades are automatically accepted. Acceptance of those with low general average will be dealt with on a case to case basis.
c. Grade 6 National Achievement Test (NAT) result
d. Original PSA Birth Certificate
e. Income Tax Return or Barangay Certification for Tax Exemption (for ESC applicants)
2. Elementary Graduates of Other Schools
Elementary graduates of other schools are given the opportunity to enroll if the following requirements are met/submitted:
a. A copy of the latest Form 138/SF9 (Report Card)
b. Letter of Recommendation or Certification of Good Moral Character from the applicant’s principal.
c. Baptismal and Confirmation Certificates
d. Original PSA Birth Certificate
e. Income Tax Return or Barangay Certification for Tax Exemption (for ESC applicants)
f. Satisfactory grade in conduct or its equivalent.
g. Applicants must pass the Qualifying Entrance Test.
h. Applicants must pass the interview to be conducted by JHS principal if required.
i. Applicants who belong to another religion will be accepted after an interview with the parents and only if he/ she is willing to sign an Agreement containing the rules to be followed during religious and spiritual activities as scheduled by the school.
Old Students
Old Students who still desire to pursue their junior high school education in SJASQ should comply with the following:
a. Submission of Form 138/SF9 (Report Card)
Note: Report card will be issued only after the student has completed his/her clearance
b. No failing grade in any subject
c. Satisfactory grade in conduct or its equivalent.
d. Credentials of students who had taken summer class/es.
Transferees
Applicants who come from other schools are given the opportunity to enroll in SJASQ provided they submit and meet the following requirements:
a. A copy of the latest Form 138/SF9 (Report Card)
b. Letter of Recommendation or Certification of Good Moral Character from the applicant’s school head/ principal.
c. Baptismal and Confirmation Certificates
d. Original PSA Birth Certificate
e. Satisfactory grade in conduct or its equivalent.
f. No failing grade/ no back subject
g. Applicants must pass the Qualifying Entrance Examination.
h. Applicants must pass the interview to be conducted by the JHS Principal
i. Applicants who belong to another religion will be accepted if the requirements are complied with and he/she is willing to sign a separate waiver containing the rules to be followed during religious and spiritual activities sanctioned by the school.
j. If the applicant is an ESC grantee, s/he must submit a Certification from the previous school regarding the ESC grant received by her/him.
k. For Grade 10 transferees – Form 137 and other credentials must be submitted not later than January of the school year.
Senior High School
Enrolling in Grade 11
Academic and conduct requirements:
(1) Applicants to Arts, Social Sciences and Humanities (ASSH) and Business and Entrepreneurship (B & E) clusters must have a general average of not lower than 80% and with at least satisfactory conduct grades.
(2) Applicants to Science, Technology, Engineering and Mathematics (STEM) cluster must have a general average of not lower than 85% and at least satisfactory conduct grades. Also, the final rating in Science and Mathematics subjects must be at least 83%.
(3) Acceptance of those with lower general average will be dealt with on a case-to-case basis.
Grade 10 Completers of SJASQ
In addition to the above requirements, in order to be accepted to Grade Eleven, each applicant should submit the following requirements:
a. Grade Ten Form 138/SF9 (Report Card)
b. National Career Assessment Examination (NCAE) result
c. Original PSA Birth Certificate
Grade 10 Completers from Other Schools
Grade 10 completers from other schools are given the opportunity to enroll if the following requirements are met/ submitted, in addition to academic and conduct requirement stated above:
a. A copy of the latest Form 138/SF9 (Report Card)
b. Letter of Recommendation or Certification of Good Moral Character from the applicant’s principal.
c. Baptismal and Confirmation Certificates
d. Original PSA Birth Certificate
e. Income Tax Return or Barangay Certification for Tax Exemption (for Voucher Program Beneficiary non-ESC applicants).
Note: Non-ESC VPB applicant must apply through the Online Voucher Application Portal (OVAP). Grade 10 completers from public Junior High Schools will be assisted to become Voucher Program Beneficiaries.
f. ESC Certification from previous school (for private school Grade 10 completers only)
g. Applicants must pass the Qualifying Entrance Test.
h. Applicants must pass the interview to be conducted by the SHS principal if required.
i. Applicants who belong to another religion will be accepted after an interview with the parents and only if he/she is willing to sign a separate Agreement containing the rules to be followed during religious and spiritual activities as scheduled by the school.
Old Students
Old Students who still desire to pursue their senior high school education in SJASQ should comply with the following:

Note: A student is enrolled if s/he has submitted her/his Form 138/SF9 and other similar transfer documents and requirements, has made payment with the school and has duly started attending classes. The enrolment of any applicant from a private school course which has neither government permit nor recognition shall not be valid.
